On form walls personnel shall use patented construction form tie‐off attachments or lifelines to
secure their safety lanyards.
17.9 Rigging/Crane Assembly and Dismantling
Managing safety risks in crane assembly‐dismantling operations present a special challenge to the
100% fall prevention objective. However, through planning and implementation by Subcontractor,
maximum protection can be achieved.
Fall protection can be obtained during these operations through the use of methods including but
not limited to retractable lifelines, safety harnesses, dual lanyards and by minimizing movement in
elevated areas by using ladders and personnel lifts.

18. MOTOR VEHICLE OPERATION
All personnel shall survey needs for construction equipment, vehicles and buses/personnel transport and
select and provide equipment adequate to safely perform the work. Each supervisor must Include In his HSE
program a procedure relating to construction equipment/vehicle inspection, Operator training, operation
and maintenance that is consistent with the Contractor/Client requirements as contained in the Project Site
HSE Plan as well as manufacturer's recommendations. All designated personnel shall be experienced,
licensed, and/or properly trained as appropriate.
18.1 Motor Vehicle and Heavy Equipment Use Rules
The parking brake must be set whenever the vehicle is parked. Equipment parked on an incline must
have the wheels chocked.
NO employee is allowed to ride in the back of loaded pickups, bed of dump trucks or on any piece of
heavy equipment except where proper seating is provided.
NO employee is allowed to sit or rest under or directly in front of or behind the wheels, bucket,
tracks, rollers, blades, rippers, etc. of any vehicle or heavy equipment.
All raised hydraulics is to be securely blocked while repair work is being accomplished to prevent
their release and subsequent fall.
Do not back‐up any vehicle or equipment when the view of the rear is obstructed unless:
It is equipped with an operation back‐up alarm which is audible above the surrounding noise for a
distance of 200 feet.
A ground guide observer signals that it is safe to do so.
All mobile heavy equipment must be equipped with an audible reverse signal alarm. Such alarms
must have a sound pressure level of 112 Dba +4 dba . the alarm must be actuated through the
equipment’s existing back up light circuits or lever / action switch from the vehicle transmission.
All engines of vehicles and heavy equipment must be turned off during refueling operations.
19. UNSAFE CONDITIONS/SAFETY SUGGESTION REPORT PROCEDURES
When an unsafe condition is observed, or there is a suggestion for how to improve safety, the originator
completes the first section of the report.
The originator should take the appropriate steps to implement the suggestion or correct the
condition that he/she has the ability to do.
