This action should be documented under Immediate Action Taken.
Any further action that the originator can recommend to correct the situation should also be
entered.
The originator sends the report to his/her immediate supervisor. A copy is routed to the affiliated HSE
Supervisor, and a copy retained by the originator, if desired.
The supervisor takes immediate action, as required, to address the concern/suggestion and document it on
the report. If the supervisor's action does not completely resolve the issue, subsequent action or
recommended actions are documented.
The supervisor sends the report to the affiliated HSE Supervisor, if required.
If further action is required, the HSE Supervisor assigns responsibility for each identified action, with a
projected completion date. This is documented under Action/Responsibility Assignments.
The HSE Supervisor and/or immediate supervisor review the report with the originator to explain the steps
that are assigned. The originator should initial in the margin next to the Action/Responsibility Assignments
box.
When items are completed, the completion date is entered in the appropriate space, and the person who is
responsible initials in the space next to the completion date. When all action is complete, the immediate
supervisor reviews the report with the originator, giving any explanations that are necessary so the
originator understands why his/her report received the action it did.
♦ The originator checks the box under Final Review of Action.
♦ Space is provided for the originator to make any comments that he/she feels appropriate and signs
the report.
♦ The completed report is sent to the Sama Global HSE Supervisor for retention and review, as required.

19.1 Roles/Responsibilities
The report assigns responsibility for the HSE process to all employees, beginning with the employee
who observes an unsafe condition or is able to suggest a way the HSE process can be improved.
It is the responsibility of supervisors and management to respond to their team members’ safety
concern.
The employee who observes an unsafe condition, or who has a suggestion for improvement of the
HSE process initiates the Unsafe Condition/Safety Suggestion Report. This employee is responsible
for taking the necessary steps to immediately address the issue or condition, as well as providing
suggested actions for long‐term resolution.
The employee’s immediate supervisor addresses the suggestion(s) provided in the report.
The supervisor should resolve the issue completely, if possible.
In this case, the supervisor should close the report by reviewing it with the employee.
If the supervisor is unable to close out the report, the responsibility shall be moved to the next
level of management.
The project manager and/or HSE supervisor is responsible for resolving all actions the immediate
supervisor is unable to complete.
