15. PERMITS

 Work permits and flame Permits are required on a daily basis.

 Close proximity permits will be required daily for use of construction equipment over, or within 15
feet of, any overhead electric power line, cable tray, process pipe, or service pipe. Supervisors shall
fill out the necessary permit forms covering the following day's work and submit them daily for
Sama Global HSE's approval.

 Confined space entry permits will be required daily for any work inside any, tank, vessel, sewers, lift
station pits or other confined space (i.e., any area which must be entered through a restricted
opening, such as tanks, vessels, ductwork, vessel skirts, manholes, etc).

♦ An excavation permit is required for each area to be excavated. Sama Global or Client will provide
supervisors with a copy of the permit before project commences excavation work in the area
covered by the permit. employees shall comply with all restrictions or any action required prior to
commencement of the work or stated in the permit (if permit required).

16. MATERIAL SHIPMENT AND STORAGE

Project manager shall coordinate shipments with Sama Global HO to avoid interfering with or site
operations. Reasonable ground space for storage of materials, office, etc, will be made available to PM as
near the work area as possible.

Office Buildings and Trailers Office building shall:
 Be blocked up in their final position and tied down with hurricane straps.
 Have fire extinguisher rated 10 A:B:C or higher.
 Be heated by electrical power only.
 Not be used for bulk storage of flammable or toxic materials.
 Main office building or trailer shall have a weatherproofed notice, prominently posted, indicating
the person's name and telephone number to contact in case of emergency at any time.
 No one shall enter a trailer that has been disconnected from the truck until additional blocking has
been placed to support the pin end of the trailer in case the "fifth wheel" fails.
 Buildings and trailers shall be provided with steps with handrails and a landing at the top of the
steps to provide safe access thereto.

Prohibited items include:
 Firearm*, drugs of abuse, and alcohol are prohibited on Site.
 Asbestos‐bearing materials and refractory ceramic fiber insulation are also prohibited.
 All people are entitled to work in a respectful work environment. Language, pictures, and items that
may be offensive to specific groups or individuals are not allowed on the Site. Employees who fail to
observe the right to a respectful environment and may be denied access to the job site. Problems or
questions can be directed to your Sama Global HSE Representative.
