11.15 Assured Grounding and Ground Fault Protection
All extension cords must have appropriate plug and be in good condition. All cords must be
inspected and approved by electrical department before use. Electrical dept will tag the cord for
approved use and duration of use. At no time will bare wire be inserted into any electrical outlet.
Portable electric tools and equipment shall be unplugged when not in use.
A Ground Fault Circuit Interrupter (GFCI) shall be used on portable electric tools when used in wet
conditions.

11.16 Gas Cylinders
Compressed gas cylinders shall be properly secured on two‐wheel hand trucks designed for this use
and brought into buildings only as needed and removed as soon as work is completed or tanks are
emptied. At the end of each shift, all cylinders shall be broken down with regulators removed and
protective caps screwed down hand‐tight. Cylinders stored outside of bottle carts shall be secured
with a non‐combustible material such as wire rope or chain.
11.17 Tool Inspection
All portable tools and equipment, including personal protective equipment, shall be properly stored
and maintained in safe working order and are subject to inspection at any time while on the
worksite. Company retains the right to prohibit or restrict the use of tools and equipment
determined to be in unsafe working condition. Any tools or equipment found to be or suspected of
being defective or unsafe will not be used.
Example* of faulty tools: chisels with mushroomed heads, hammers with loose or split handles and
guard* missing on saw* or grinders.
11.18 Projecting Materials
Reinforcing steel or similar material projecting above horizontal surfaces shall be capped or
otherwise protected to prevent the possibility of injuring personnel.
11.19 Personnel in Elevated Structures
Site Supervisor shall provide means of access and egress adjacent to the work area for personnel
working in elevated structures. Ladders or similar means of access shall be placed 50 feet of center
to surface for proper access and egress to elevated work.
11.20 Control of Hazardous Energy (Lockout/Tagout)
Site Supervisors shall comply with Health, Safety and Environmental Reference Manual ‐ Control of
